Does NOT fall out.
Everyone who has complained that it fell out, you're just too loose to keep it in. My man LOVES when I break this out! It stays in place, for the whole ride, did not melt, vibrator works EVERY time.

Not worth the money
Difficult to use the first time and the vibrator stopped working the second time. Nearly impossible for my GF to keep in her. Stored it in a bag next to another toy and this one got deformed.

Meh.
So-so. It's ok - a little pricy for what you get. It needs a little reinforcement in the "L" to make it more effective. The wiring to the mini bullet broke after only about ten uses.

we named him barney, hes a pretty purple color, but he sucks in bed. dont buy this! the vibrating thingy came unglued and the wires unattached themselves the first time we used it.
